Operational Plan 2015/16
This report sets out Wyre Forest Clinical Commissioning Group’s plans for 2015/16. We are producing this report to demonstrate how we intend to make progress towards the Wyre Forest CCGs overall vision to ‘bring together local people, GPs and other clinical professionals to improve the quality and experience of health care for our patients’ . It also demonstrates how we aim to make progress against the plans we set out with our partners in the Worcestershire 5 Year Health and Care Strategy.
This plan aims to provide our residents, our stakeholders, our partners and our members with a clear picture of the improvements and changes we want to make to health and care services over the next year, as well as providing an outline of what we want health and care services to look like by 2020.

NHS Wyre Forest CCG Corporate Objectives 2016/17
Our Corporate Objectives set out what we plan to focus on during 2016/17, helping the organisation to prioritise resources to a number of initiatives which we believe will make the greatest difference to the community.
The objectives are based around four key strategic themes:
- Improving Care & Quality
- Development of Organisational Structures and Infrastructure
The 16/17 corporate objectives comprise of common objectives across all 3 Worcestershire CCGs; with areas of local focus highlighted in blue. This will help ensure that all 3 CCGs are focussing efforts on the key healthcare challenges within Worcestershire.

5 Year Health & Care Strategy for Worcestershire
This document outlines the initial view of the Worcestershire five year strategy, as required by Everyone Counts: Planning For Patients 2014/15 to 2018/9. This version is near complete, but some updates will be required before it can be finalised. Process for developing the 5 year strategy The significant steps in the process for the development of this strategy have been:
• An initial presentation to the Health and Well Being Board on 11th March 2014. At this meeting we sought the Health and Well Being Board endorsement to the process and timeline so that a formal first draft could be submitted to NHS England on 4th April 2014.
• The Strategic Visioning Events on 8th and 9th April where all key stakeholders in the Health and Social Care economy came together to discuss challenges and future plans.
• A presentation to the Health and Well Being Board on 13th May to share the initial draft that was submitted on 4th April along with a proposed timeline for amending the next version to be submitted on 20th June.
• A facilitated discussion at the development session of the Health and Well Being Board on 18th June, following which the Board approved the Strategy for submission to NHS England on 20th June.
• The final strategy will be presented to the Health and Well Being Board on 22nd July 2014 so it should not be considered a final strategy until this process has passed.

Worcestershire Better Care Fund Plan
Worcestershire has a long history of partnership working and a strong vision and ambition to deliver improved services to the people of Worcestershire through better integration. Our Section 75 agreement for 2014/15 covers £206,182,537 of services for Adults (including mental health, learning disability and older people's services). The Integrated Commissioning Unit (formerly the Joint Commissioning Unit) is hosted by Worcestershire County Council and commissions services funded jointly by the Council and the three Worcestershire Clinical Commissioning Groups through the section 75 agreement for the people of Worcestershire.
The publication of ‘Integrated Care and support: our shared commitment’ in 2013 was received with eagerness by senior leaders, patient and carer representatives and staff across all our organisations, we are absolutely determined to find ways to work across and beyond organisational and professional boundaries to deliver personalised care.

Worcestershire Health and Wellbeing Strategy 2013-16
The Worcestershire Health and Wellbeing Strategy is based on the findings of the Worcestershire Joint Strategic Needs Assessment and sets out Worcestershire Health and Wellbeing Board's vision, priorities and goals.
The priority areas are:
Older people and management of long term conditions
Mental health and wellbeing

Joint Strategic Needs Assessment
Local authorities and local health services are required to undertake Joint Strategic Needs Assessments of health and well-being. This work is a continuous process of assessment designed to inform decisions made locally about what services are commissioned. The core aim is to improve the public's health and reduce inequalities.
Findings from the Joint Strategic Needs Assessment and public consultation are used to produce a Joint Health and Well-being Strategy through Worcestershire's Health and Well-being Board.
